The 61-year-old, known for his work in films such as Saving Private Ryan and Black Hawk Down, never regained consciousness after he collapsed at his Los Angeles home on 18 February.
According to his manager, actor Tom Sizemore died in his sleep in a hospital weeks after suffering a brain aneurysm.
“I am deeply saddened by the loss of not only a client but also a great friend and mentor of almost 15 years,” Charles Lago said.
Tom was one of the most genuine, kind, and generous people I had the pleasure of knowing.
“His courage and determination through adversity were always an inspiration to me.
“The last few years were great for him, and he was getting his life back on track.

“He adored his sons and family.
“I will greatly miss my friend Tom Sizemore.”
Sizemore, 61, collapsed at home in Los Angeles before being taken to hospital by ambulance after suffering a brain aneurysm as a result of a stroke in the early hours of February 18.
He had been in critical condition, in a coma, and in intensive care since then.
‘Support and love messages’
Mr Lago said he never regained consciousness and died in his sleep with his brother Paul and twin boys Jayden and Jagger, both 17, by his side.
Mr. Lago stated: “The Sizemores have been comforted by the hundreds of messages of love and support sent to their son, brother, and father.
“They have requested privacy during this difficult time, and I respectfully request that those requests be granted.

Mr Lago said there will be a private cremation service for the family, followed by a larger “celebration of life event” in a few weeks.
Licence to Kill and The Goonies actor Robert Davi paid tribute to Sizemore on social media, saying he was “saddened to hear” the news.
“His family has my prayers – Tom and I were friends I knew of his suffering & struggle with addiction – he was a terrific talent – Many families suffer from the heartbreak and tragedy of this disease and we do not do enough!!” he wrote on Twitter.

A ‘spoiled movie star’ and an ‘arrogant fool’
Sizemore rose to prominence after appearing in Natural Born Killers and Heat, before moving on to blockbusters such as Saving Private Ryan and Black Hawk Down.
However, he struggled with addiction throughout his acting career, being arrested several times for driving under the influence and drug possession.
He was also arrested for domestic violence multiple times, including once against his girlfriend Heidi Fleiss, for which he was convicted.
Sizemore was also the subject of two sexual harassment lawsuits at work in connection with the 2002 CBS show Robbery Homicide Division.
In a 2013 memoir, he described himself as a “spoiled movie star,” a “arrogant fool,” and a “hope-to-die addict” as a result of his success.
“I’ve led an interesting life, but I can’t tell you what I’d give to be the guy you didn’t know anything about,” he added.
